Cedar Ridge in Bethany is seeking a Clinical Mobile Assessor and Community Liaison to develop our Senior Adult service line at Bethany Behavioral Hospital. The case manager will serve as the marketing and outreach coordinator for our geriatric unit and will also be responsible for assessing potential patients to determine necessary services. The mobile assessor/marketing rep will serve as the liaison between hospital staff (including admissions coordinators, clinical team and psychiatrists) and outside referral sources such as nursing homes, assisted living centers, memory care facilities, hospitals and other senior adult service providers. Core responsibilities will include: developing and maintaining relationships with referral sources and providing case management services to patients before, during and after admission, prospecting potential referral sources and conducting initial and follow up outreach, communicating with hospital staff to ensure seamless and efficient admissions process, developing working report with attending psychiatrists and other hospital staff as necessary to help mitigate and assess the psychiatric and medical complexities of individual patients, and networking with senior adult services providers and other mental health professionals.
The Case Manager will report to the Director of Business Development and will collaborate with the Director as well as clinical and nursing staff to develop and implement effective marketing and outreach strategies across all lines of service within the hospital. Because the Mobile Assessor/Marketing Rep will be charged with mobile assessments in nursing homes and other referral source facilities, the candidate will be a licensed mental health clinician (LCSW, LPC, LMFT) or under supervision for one of the aforementioned licensures.
